All registered students are required to have an Incoming Medical Health History/Immunization Verification form on file with College Health Services.

A complete list of current prescription medication – particular those taken for a prolonged period of time, should be noted on this form, in case of emergency.

Documentation from your health provider (registered medical doctor or public health facility) along with the immunizations verification form must be sent to the Student Affairs Department to complete your registration process.

Note additional immunization requirements may be required for certain programmes and for participation in inter collegiate sports.

If you have a medical contraindication to any of BCC immunization requirement, submit a letter of explanation signed by your medical provider or enter this information on your medical form as indicated.

• Please be aware that if a communicable disease outbreak occurs, those who are not properly vaccinated or have a waiver due to a personal or religious belief (supported by an Affidavit) will be the first to be asked to leave campus for their own safety.
